Localized/small tumors (FIGO stage I) have significantly better survival than tumors with distant spread (Typically FIGO stage IV). Cancers that generally are detected early (such as endometrial cancer after postmenopausal bleeding) have a mostly good prognosis, while ovarian cancer with few early signs are mostly diagnosed late and thus have a poorer overall survival rate. The only preventable gynecological cancer (HPV-vaccination, see Chapter Premalignant lesions of the uterine cervix) is also the only one with a good screening option (cervical smear/HPV-test, described in the Approach to the patient chapter, part Performing cervical cytology). Following mass vaccination the incidence of cervical cancer will likely decrease in the coming decades. Endometrial cancer on the other hand has displayed an increased incidence due to the general aging of the population in combination with obesity. </p> <p>The different cancers and their treatment are described in the specific sub-chapters. The Nordic countries have guidelines that differ slightly. <p>Gestational trophoblastic disease (GTD) defines a spectrum of pregnancy-related disorders from benign or pre-malignant forms of partial and complete hydatidiform mole (HM) to malignant disorders of invasive mole, choriocarcinoma (CC), placental site trophoblastic tumour (PSTT), and epithelioid trophoblastic tumour (ETT). A HM can be followed by persistent trophoblastic disease (PTD) characterized by rising or plateauing hCG values, and a need for chemotherapy to obtain remission. The cure rate in PTD is 99%, and 70-95% in CC and PSTT/ETT. <br> <br> All GTDs arise from the extra-embryonic structures of a pregnancy (placental tissue) and the lesions can be invasive, very vascular and metastatic. The most common GTD is HM, which is classified as partial or complete. HMs have a unique genetic constitution, as they have two sets of paternally inherited genes. Most complete moles are diploid and the majority of partial moles are triploid. Diploid moles arise when an empty egg is fertilized by a single sperm, that duplicates its own DNA, producing a homozygotic, androgenetic mole. A triploid mole results from over-fertilization in a normal egg by two sperm cells (Figure 19). </p> <p>Trophoblastic tumors are characterized by the secretion of human Chorionic Gonadotropin (hCG), making hCG a unique marker used in diagnosis and surveillance of the disease. </p> <p><img title="Figure 19. In Asian countries, HMs are more frequent, which may reflect dietary or genetic influences. A twin pregnancy involving a HM and a normal fetus is seen in 1:20.000–120.000 pregnancies. Just over 50% of these pregnancies succeed in a normal child. </p> <h3>Risk factors</h3> <ul> <li>Maternal age. Increased risk in women < 16 years and > 40 years.</li> <li>Previous molar pregnancy.</li> <li>Previous spontaneous abortions.</li> <li>Genetic disposition. Hereditary recessive predisposition in the mother. </li> </ul> <h3>Symptoms</h3> <ul> <li> Vaginal bleeding in early pregnancy.</li> <li>Nausea (due to high level of hCG) </li> </ul> <h3>Diagnostics and treatment</h3> <ul> <li>hCG measurement (<em>much</em> higher levels than normal pregnancy).</li> <li>Ultrasound scans will reveal a classic “snowstorm” picture, showing multiple vacuoles of 1-15 mm size.</li> <li>In the complete mole, the hydropic tissue will expand the whole uterine cavity, where as a non-viable fetus and amniotic fluid can be present in the partial mole.</li> <li>If there is no sign of molar growth involving the uterine wall, a HM can be safely evacuated. It is essential to measure hCG prior to the evacuation.</li> <li>Pathological examination of tissue.</li> <li>Preferably fresh tissue is sent for genetic analysis.</li> </ul> <h3>Follow-up after evacuation</h3> <ul> <li>Weekly hCG to exclude persistent throphoblastic disease (PTD).</li> <li>All moles are followed until hCG reaches normal values.</li> <li>Diploid moles are followed for 4-6 months.</li> <li>Contraception during the follow-up period is recommended. </li> </ul> <h2>Persistent trophoblastic disease (PTD)</h2> <p> </p> <h3>Epidemiology</h3> <p> 10-15% of molar pregnancies develop PTD. </p> <h3> Risk factors</h3> <p> Ploidy is the best prognostic factor for PTD, as the risk of PTD after a diploid mole is 16-18%, whereas after a triploid mole, the risk approaches 0. Age > 40 and initial high hCG (> 100.000) increases the risk of PTD. </p> <h3> Diagnostics</h3> <p> PTD is diagnosed when hCG: </p> <ul> <li>Rises for more than 2 weeks (3 measurements) after the primary evacuation</li> <li>Decreases less than 10% over 3 weeks (plateau during 4 measurements)</li> </ul> <p>Imaging, such as MRI of abdomen and pelvis and chest X-ray or CAT-scan, should be performed to detect any uterine lesions or metastases, most frequently seen in the lungs. </p> <h3>Treatment</h3> <p> PTD is treated with chemotherapy until hCG is normalized. A scoring system (FIGO 2000) divides patients into low risk and high risk. In low risk disease most centers give Methotrexate (MTX) as single agent treatment, whereas multi-agent treatment is given in high-risk disease. Another approach is a response-adapted regimen, where all patients start with MTX and shift to more toxic regimes in case of MTX-resistance. The total cure rate is 99%. </p> <h2> Rare tumors: gestational choriocarcinoma, PSTT and ETT</h2> <p> The frequency of gestational choriocarcinoma (CC) is 1:50,000 deliveries. PSTT and ETT are just as rare. Half of these rare tumors arise after a normal pregnancy, and half occur after a HM. The treatment is mainly chemotherapy in CC, whereas PSTT/ETT primarily is treated with surgery.
