Antistofferne kan passere placenta til det føtale kredsløb, hvor de destruerer fosterets erytrocytter, hvilket kan resultere i føtal anæmi<a title="Kumar S, Regan F, Management of pregnancies with RhD alloimmunisation. Moderen kan danne anti-D antistoffer, der via placenta kan nå fosterets kredsløb og føre til føtal anæmi</li> <li>Andre typer af blodtypeimmunisering omfatter bl.a. C, E, c, e og Kell, hvoraf c og Kell er særligt alvorlige </li> </ul> <p>I det følgende vil der blive fokuseret på immunisering med anti-D, da det er den hyppigst optrædende form for blodtypeimmunisering</p> <h3>Forekomst</h3> <ul> <li>15 % af de gravide har RhD negativ blodtype, og heraf bærer 60 % på et foster med RhD positiv blodtype </li> <li>I Danmark, hvor fødselstallet nu ligger omkring 60.000 årligt, svarer det til ca. 9.000 gravide med RhD negativ blodtype, hvoraf ca. 5.400 bærer et foster med RhD positiv blodtype </li> <li>Risiko for immunisering efter en fødsel <ul> <li>RhD immunisering i forbindelse med graviditet og fødsel forekom tidligere i ca. 1,5 % af tilfældene, svarende til ca. 100 kvinder om året, hvoraf ca. 10 var alvorlige med behov for intrauterin blodtransfusion til barnet</li> <li>Siden 2010 har alle RhD negative gravide haft tilbud om at få fosterets RhD type bestemt på en blodprøve fra moderen i uge 25 og efterfølgende behandling med [DliActiveSubstance;6334;anti-D immunoglobulin] i uge 29, hvis barnet er RhD positivt. Hyppigheden af rhesusimmunisering forventes på denne baggrund at blive reduceret til ca. 0,5 %. Foreløbige data tyder på, at hyppigheden af rhesusimmunisering i Danmark er faldende, men endelig opgørelse foreligger endnu ikke</li> </ul> </li> </ul> <h3>Ætiologi og patogenese</h3> <ul> <li>En retrospektiv undersøgelse fra Rigshospitalet i København fra 1998-2005 med i alt 390 immuniserede gravide kvinder viste, at anti-D er det hyppigste antistof ved immunisering (47 %), fulgt af anti-K (15 %). Transfusion 2009;49: e96-102" data-type="journal-reference" href=" ">9</a></li> <li>Eksponering <ul> <li>Der passerer føtale celler over i moderens cirkulation med stigende hyppighed i løbet af graviditeten, men normalt er det volumen, som passerer, ikke nok til at stimulere moderens immunsystem </li> <li>Hos en RhD negativ kvinde (har ikke D-antigenet) skal der som regel to eksponeringer med rhesus antigen til for at sensibilisere kvinden, dog kan en større føto-maternel blødning i samme graviditet også forårsage rhesus immunisering. Eksponering kan ske ved tilfældige blødninger i graviditeten, men sker især under fødslen</li> </ul> </li> <li>Hæmolyse <ul> <li>IgG antistoffer (anti-D), som produceres af moderen som følge af eksponering for fosterets RhD positive erytrocyter, kan passere placenta og sætte sig fast på fosterets erytrocytter og forårsage hæmolyse </li> <li>Ved mild hæmolyse kan fosteret kompensere ved at øge erytropoiesen.
