Am J Respir Crit Care Med 2022;206: e7-e41">6</a></li> <li> Desuden er der blandt de ILS flere histologiske undertyper, hvoraf de vigtigste er: <ul> <li> UIP: usual interstitiel pneumonitis, som er det typiske billede ved den alvorlige ILF </li> <li> DIP: deskvamativ interstitiel pneumonitis, hvor alveolerne opfyldes med mononuklæere celler </li> <li> COP: kryptogen organiserende pneumonitis også kaldet BOOP (Bronchiolitis obliterans organiserende pneumoni), hvor der opstår peribronchiolær konsolidering </li> <li> NSIP: Nonspecific interstitial pneumonitis, hvor der er diffus interstitiel inflammation eller fibrose, hvor læsionerne i modsætning til ILF har samme modenhedsgrad </li> <li> RB-ILD: Respiratory Bronchiolitis in lnterstitial Lung Disease: ligner radiologisk og patologisk DIP </li> <li> AIP: Akut interstitiel pneumonitis er en sjælden og akut indsættende som har et klinisk/radiologisk billede som ved ARDS </li> <li> LIP: Lymfocytær interstitiel pneumonitis er sjælden og karakteriseret ved infiltration af lymfocytter i store ansamlinger </li> </ul> </li> <li> De vigtigste ILS med kendt årsag er: <ul> <li> Erhvervs- eller miljøbetinget sygdom: herunder <a href="~/link.aspx?_id=F41E9CFC9FCA4C82B4631AE3C76303A2&_z=z">allergisk alveolitis</a> og <a href="~/link.aspx?_id=5192DF4DFEFE47F5B2407E427E34BE8D&_z=z">pneumokonioser</a> som silikose eller <a href="~/link.aspx?_id=4717DC1C35444748BAE1BABAD950DD58&_z=z">asbestose</a> </li> <li> Behandlingsinduceret sygdom, f.eks. strålefibrose eller reaktioner på lægemidler </li> <li> Interstitiel lungesygdom som led i rheumatisk sygdom </li> </ul> </li> </ul> </li> <li> <a href="~/link.aspx?_id=03D3C24F4A1C45EEBE4F0C41637D9447&_z=z">Sarkoidose</a> kan ledsages af interstitiel lungesygdom og er omtalt i et særskilt kapitel </li> </ul> <h2>Diagnose</h2> <h3>Diagnostiske kriterier</h3> <ul> <li>De vigtigste kriterier for at stille diagnosen Idiopatisk Lungefibrose (ILF)<a href="%20" title="Lynch DA et al, Diagnostic criteria for idiopathic pulmonary fibrosis: a Fleischner Society White Paper. Lancet Respir Med 2018;6: 138-53" data-value-piped="Lynch DA et al|Diagnostic criteria for idiopathic pulmonary fibrosis: a Fleischner Society White Paper|Lancet Respir Med|2018|6|138-53|" data-url="reference-link">7</a> <ul> <li>Karakteristiske retikulo-nodulære forandringer på røntgen af thorax</li> <li>Bikubetegning bibasalt subpleuralt ved CT thorax</li> <li>Restriktiv nedsættelse af lungefunktionen</li> <li>Udelukkelse af andre årsager til interstitiel lungesygdom</li> <li>Alder oftest over 50 år</li> <li>Snigende funktionsdyspnø uden anden årsag</li> <li>Symptomvarighed på over 3 måneder</li> <li>Fin inspiratorisk krepitation basalt på begge lungebagflader</li> <li>Tilstedeværelse af neutrofile granulocytter i bronkieskyllevæsken (BAL)</li> <li>Tilstedeværelse af UIP ved lungebiopsi <ul> <li>Udtalt fibrose/arkitektonisk distortion</li> <li>Bikagetegning overvejende subpleuralt/ paraseptalt</li> <li>Tilstedeværelse af heterogenicitet </li> <li>Tilstedeværelse af fibroblast foci</li> <li>Fravær af vaskulitis og granulomatøs inflammation i lungebiopsi, som tyder på anden patogenese</li> </ul> </li> </ul> </li> </ul> <h3>Sygehistorie</h3> <h4>Idiopatisk lungefibrose (ILF)</h4> <ul> <li>Begynder ofte snigende så debuttidspunkt kan være svært at fastsætte</li> <li>Funktionsdyspnø som varer mere end 6 måneder</li> <li>Tør hoste</li> <li>Andre lungesymptomer og almensymptomer i form af feber eller vægttab er ikke almindelige</li> </ul> <h4>Andre typer interstitiel pneumonitis</h4> <ul> <li>Både deskvamativ pneumonitis og cryptogen organiserende pneumonitis begynder ofte mere akut og udvikles hurtigt over få måneder</li> <li>Ofte influenza eller pneumonilignende symptomer ved debut eller kort tid før</li> <li>Almensymptomer med sygdomsfølelse og subfebrilia</li> </ul> <h3>Kliniske fund</h3> <h4>Idiopatisk lungefibrose</h4> <ul> <li>Urglasnegle og trommestikfingre ses hos 25-50 % <br> <img style="width: 200px; height: 150px;" alt="Trommestikfingre" src="8380F3EAB36C40B185DC2B16381130BA-200-150" height="150" width="200"></li> <li>I starten normal perifer iltmætning (saturation) i hvile</li> <li>Saturation falder ved hurtig gang mere end 3 %</li> <li>Ved auskultation af lungerne høres fin krepitation basalt på begge bagflader</li> </ul> <h4>Andre typer af lungefibrose</h4> <ul> <li>Som regel er der ikke urglasnegle eller trommestikfingre</li> <li>Mange er hypoksiske på diagnosetidspunktet</li> <li>Ved auskultation af lunger kan der høres fin krepitation</li> </ul> <h3>Supplerende undersøgelser i almen praksis</h3> <ul> <li>Blodprøver er uspecifikke</li> <li>Hvis man råder over en saturationsmåler, kan saturation måles i hvile og ved/efter fysisk anstrengelse f.eks. gang til 2. sal</li> </ul> <h3>Andre undersøgelser hos specialist</h3> <h4>Røntgen af thorax</h4> <ul> <li>Ved idiopatisk lungefibrose ses retikulo-nodulære forandringer basalt evt. med cystedannelse</li> <li>Ved andre typer af interstitiel pneumonitis er forandringerne mere diffuse og mindre specifikke</li> </ul> <h4>CT af thorax</h4> <ul> <li>CT af thorax med højopløsningsteknik (High resolution CT: <a href="~/link.aspx?_id=475E3A0A0E6E4DB2B3D990EAF23E94EE&_z=z">HRCT</a>) er en meget vigtig undersøgelse</li> <li>Ved klassisk idiopatisk lungefibrose er forandringerne så karakteristiske, at man i dag som regel afstår fra lungebiopsi for at stille diagnosen. Der ses typisk bi-basal, subpleuralretikulering med bikagetegning og traktionsbronkiektasier</li> <li>Ved deskvamativ interstitiel pneumonitis og akut interstitiel pneumonitis ses ofte matglastegning med kun lidt fibrose og ingen cystedannelse</li> <li>Ved kryptogen organiseret pneumonitis (COP/BOOP) ses subpleural og peribronchial konsolidering og evt. luftbronchogram</li> </ul> <h4>Lungefunktionsmåling</h4> <ul> <li>I de fleste tilfælde ser man <ul> <li>En restriktiv lungefunktionsnedsættelse, dvs. små stive lunger</li> <li>Diffusionskapaciteten nedsættes tidligt, og i sene faser af sygdommen kan den være nedsat helt ned til 20 % af forventet værdi</li> <li>Senere tilkommer nedsættelse af total lungekapacitet ofte ned til 50 % af normalværdien. Vitalkapacitet og FEV<sub>1</sub> er tilsvarende nedsat</li> <li>FEV<sub>1</sub>/FVC ratio er ofte større end 0,9, men i enkelte tilfælde kan der også ses et obstruktivt mønster</li> </ul> </li> </ul> <h4>Bronkoskopi</h4> <ul> <li>Bronkoskopi med undersøgelse af bronkieskyllevæske (BAL) og cryobiopsi og i få tilfælde transbronkial lungebiopsi</li> <li>Undersøgelse af bronkielskyllevæske viser typisk mange neutrofile ved idiopatisk lungefibrose</li> <li>Ved andre former for interstitiel lungesygdom er der overvejende mange lymfocytære celler eller eosinofile granulocytter</li> <li>Transbronkiale biopsier er små og er derfor ofte ikke konklusive, men kan dog være diagnostiske, hvis de interstitielle forandringer skyldes carcinose eller sarkoidose</li> <li>Transbronkiale kryobiopsier er en nyere teknik, hvor der opnås lidt større biopsier, og dermed større diagnostisk udbytte. Men det er også et mere invasivt indgreb med flere komplikationer</li> <li>Der er i dag international konsensus om, at det ikke er nødvendigt at tage lungebiopsi ved typiske symptomer og CT fund forenelige med idiopatisk lungefibrose<a href="%20" title="Lynch DA et al, Diagnostic criteria for idiopathic pulmonary fibrosis: a Fleischner Society White Paper.
