Clin Rheumatol 2018;37: 869-874">2</a></h2> <h3>Diagnostiske kriterier</h3> <ul> <li>Der er ingen etablerede diagnostiske kriterier</li> <li>Reaktiv artritis eller postinfektiøs artritis er en klinisk diagnose</li> </ul> <h3>Sygehistorie</h3> <ul> <li>Sygdomsbilledet varierer fra næsten asymptomatisk sygdom til akut indsættende systemisk sygdom med almen symptomer, mono- eller oligoartrit og ekstra-artikulære manifestationer herunder øjeninvolvering</li> <li>Omtrent halvdelen kan berette om en sandsynlig udløsende infektion 1-3 uger før udbruddet af artritten. Resten, særligt kvinder, har sandsynligvis haft en subklinisk infektion. Det kan være vanskeligt at etablere diagnosen i den sidste gruppe</li> <li>Reaktiv artritis er ofte selvlimiterende men kan være recidiverende og blive kronisk</li> </ul> <h4>Symptomer</h4> <ul> <li>Almensymptomer som sygdomsfølelse, træthed, feber, vægttab og nedsat appetit er sædvanlig i den akutte fase </li> <li>Mono-eller oligoartritis oftest med involvering af store led på underekstremiteterne, men mindre led og led på overekstremiteterne kan også rammes. Hofteled kan være involveret og er sværere at undersøge for. Ofte har patienten udadroteret hofte i hvile og får smerteforværring ved indadrotation ved klinisk undersøgelse</li> <li>Lænde- og eller bækkensmerter opleves af ca. hver fjerde patient og kan skyldes inflammation af sakroiliaca led (sakroiliitis) eller ligamenter langs columna </li> <li>Inflammation i sternoklaviculærled og sternocostalleddene giver ofte lettere symptomer, sidstnævnte som stivhed/ømhed ved dyb inspiration</li> <li>Smerter ved senetilhæftninger er ganske hyppig, f.eks. svarende til akillessenen og fascia plantaris</li> <li>Andre symptomer <ul> <li>Fra hud og slimhinder er ligeledes hyppigt forekommende</li> <li>En triade er uretritis, artritis og konjunktivitis <ul> <li>Uretritten kan variere fra asymptomatisk til svær med ledsagende prostatitis</li> </ul> </li> <li>Ved venerisk årsag har omkring en tredjedel af mændene <a href="~/link.aspx?_id=728A2F6FEDF24DA3A44CA274E9A2B455&_z=z">balanitis</a>, hos nogle kvinder kan man finde <a href="~/link.aspx?_id=3FE398B98D7246A496B94FA63E730BC3&_z=z">cervicitis</a></li> </ul> </li> </ul> <h3>Kliniske fund</h3> <h4>Generelt</h4> <ul> <li>Fuld somatisk undersøgelse bør foretages for at afsløre evt. udløsende infektion</li> </ul> <h4>Ledstatus</h4> <ul> <li>Typisk findes et eller flere meget hævede, ømme perifere led, som regel med udtalt bevægelsesindskrænkning</li> <li>Ledstatus bør også omfatte ryggen med tanke på sakroilitis</li> <li>Daktylitter kan påvises hos nogen (pølseformet hævelse af fingre og tæer pga. entesopati, periostitis og småleds-inflammation)</li> </ul> <p><img alt="Foto af daktylitter, her hævelse af tæer." src="142CC2B477184B1391218F8E1245610E-200-141" text="Foto af daktylitter, her hævelse af tæer." style="width: 200px; height: 141px;" title="Foto af daktylitter, her hævelse af tæer." height="141" width="200"></p> <h4>Andre organfund</h4> <ul> <li>Hud og slimhinder <ul> <li>Orale sår ses ofte og kan være smertefri</li> <li>Circinat balanitis, en smertefri erytematøs læsion på glans penis, findes hos omtrent 20 % af mænd med reaktiv artritis</li> <li>Hos kvinder kan en cervicitis være udtalt med vaginal udflåd</li> </ul> </li> <li>Keratoderma blennorrhagica <ul> <li>Et typisk, men sjældent fund, er et stærkt skællende udslæt på fodsålerne med elementer af sterile pustler og er vanskelig at skelne fra pustulosis palmoplantaris</li> </ul> </li> <li>Øjensymptomer er ikke sjældne og kan indbefatte alt fra asymptomatisk konjunktivit til aggressiv uveitis</li> <li>Andre sjældnere organfund kan være fra hjertet, lungerne, nervesystemet</li> </ul> <h3>Supplerende undersøgelser i almen praksis</h3> <ul> <li>Hos en patient med nyopstået ledhævelse bør man overveje henvisning til reumatologisk vurdering. Dette bl.a. pga. differentialdiagnostiske overvejelser. Ved monoartrit og mistanke om septisk artrit skal patienten altid vurderes akut. I almen praksis kan man starte udredning i form af generel blodstatus, evt. bloddyrkning samt overveje kontrol af gigtfaktoren anti-CCP målt i blodet. Desuden kan man overveje at undersøge for kønssygdomme samt virale og gastrointestinale infektioner afhængigt af symptomer</li> </ul> <h4>Generel blodstatus</h4> <ul> <li>Hæmoglobin, CRP, leukocyttal + differentialtælling, kreatinin, ALAT, p-urat <ul> <li>CRP kan være trecifret</li> <li>Som ved anden længerevarende inflammation kan der udvikles sekundær anæmi i forløbet af tilstanden</li> </ul> </li> </ul> <h4>Infektionsserologi</h4> <ul> <li>Hvis det vurderes relevant kan man overveje serologiske undersøgelser for streptokokker, gonokokker, virusinfektioner som f.eks. parvovirus B19 <a href="%20" title="Mathew AJ, Ravindran V, Infections and arthritis. Dog er det væsentligt at påvise en evt. kønssygdom, så denne kan blive behandlet. </li> </ul> <h3>Håndtering i almen praksis</h3> <ul> <li>Blodprøvestatus og undersøgelse for mulig infektion</li> </ul> <h3>Råd til patienten</h3> <ul> <li>Oftest en selvlimiterende tilstand som hyppigt opstår efter en infektion hos genetisk disponerede personer</li> <li>Reaktiv artritis smitter ikke</li> <li>Er ikke arvelig, men ses dog hyppigere i nogle familier</li> </ul> <h3>Medicinsk behandling</h3> <p>Første valgs præparatet ved reaktiv artritis er NSAID præparater såfremt patienten tåler disse. Ved sarkroiliitis er dette ligeledes førstevalgspræparatet. Man kan supplere med analgetisk behandling i form af </p> <ul> <li>[DliActiveSubstance;6381;paracetamol] tbl. 500 mg: 2 tbl. 3-4 gange dagligt</li> </ul> <p>Korterevarende behandling med [DliActiveSubstance;5892;prednisolon] kan desuden være indiceret.</p> <p>Såfremt patienten har vedvarende artrit kan man i reumatologisk regi vælge at supplere med immunhæmmende behandling:</p> <ul> <li><a href="https://pro.medicin.dk/Medicin/Indholdsstoffer/688">methotrexat</a> </li> <li>[DliActiveSubstance;5668;sulfasalazin] </li> <li>biologisk behandling med f.eks. <a href="https://pro.medicin.dk/Laegemiddelgrupper/Grupper/315408">TNF-hæmmere</a></li> <li>Intraartikulære injektioner med <a href="http://pro.medicin.dk/Laegemiddelgrupper/grupper/171050">kortikosteroid</a> kan give effektiv kontrol over sygdommen i individuelle led. En del af tilfældene er formentlig milde</li> <li>Urogenitalt udløst reaktiv artritis ses først og fremmest hos mænd (85 %), men underdiagnosticering af kvindelig urogenital infektion, specielt med klamydia, kan være forklaringen </li> <li>Reaktiv artritis efter tarminfektion er lige fordelt på mænd og kvinder </li> <li>60-80 % af patienterne med reaktiv artritis er HLA-B27 positive</li> </ul> <h3>Ætiologi og patogenese</h3> <ul> <li>En lang række agens kan associeres med reaktiv artritis, de hyppigste er patogene tarmbakterier som <ul> <li>Shigella </li> <li>Salmonella </li> <li>Yersinia </li> <li>Campylobacter</li> </ul> </li> <li>Og urogenitale mikrober som <ul> <li>Klamydia </li> <li>Gonokokker (som også kan give septisk artritis)</li> </ul> </li> </ul> <h4>Patofysiologi</h4> <ul> <li>Reaktive artritter begynder almindeligvis 1-3 uger efter en urogenital eller gastrointestinal infektion </li> <li>Artritten er som regel enten mono- eller oligoartikulær, oftest i underekstremiteternes led</li> <li>Årsagen til at nogle personer udvikler reaktiv artritis er ukendt, men der er en velkendt sammenhæng med vævstypen HLA-B27</li> <li>Ved reaktiv artritis formoder man, at immunsystemet i efterforløbet af en infektion tager fejl af overflademarkører på hhv. bakterier/virus og overflademarkører på kroppens egne celler og derfor angriber kroppens egne celler
