Ofte forbedres funktionen også en smule efter operation med skulderprotese</li> <li>Resultaterne efter skulderprotese operation følges i et nationalt dansk register</li> </ul>]]></HtmlField> <HtmlField Name="PageContent"><![CDATA[<h2>Hvad er en skulderprotese?</h2> <p> I Danmark udføres der hvert år ca. 1300 skulderprotese operationer. </p> <p>Ved operation med indsættelse af skulderprotese indsættes en ny ledskål og et nyt ledhoved, der er lavet af metal og plastik. I Danmark foretages skulderprotese operationer kun på udvalgte sygehuse. Dette har man valgt for at sikre en ensartet og høj kvalitet af behandlingen. </p> <p>Operationen foretages oftest i fuld bedøvelse, så du sover under operationen. Oftest får du også en bedøvelse af nerverne til armen (nerveblokade). Blokaden betyder, at du oftest er smertefri de første 16-24 timer.</p> <p>Efter operationen vil du oftest være indlagt et enkelt døgn, men hvis du ellers er sund og rask, kan du eventuelt udskrives samme aften.</p> <h2>Hvornår behandler man med skulderprotese?</h2> <p>Operation med skulderprotese er en stor operation og tilbydes kun i særlige tilfælde. De hyppigste årsager til, at man behandler med skulderprotese i Danmark, er </p> <ul> <li>Knoglebrud i overarmsknoglen</li> <li>Følger efter knoglebrud</li> <li>Slidgigt</li> <li>Styresene gigt eller leddegigt</li> </ul> <p>Personer der behandles med en skulderprotese er i reglen over 70 år. </p> <h3>Knoglebrud og følger efter knoglebrud i overarmen: </h3> <p>I sjældne tilfælde kan knoglebrud i overarmen ved skulderen være så komplicerede, at det er nødvendigt at behandle med en skulderprotese. Denne type brud opstår oftest hos ældre kvinder, som har knogleskørhed. </p> <p>Flere og flere personer med et knoglebrud i overarmen ved skulderen behandles uden operation. En mindre andel af disse personer har vedvarende smerter, der enten skyldes, at bruddet ikke er helet, eller at det er helet i en fejlstilling. Disse personer kan også tilbydes behandling med skulderprotese. </p> <h3>Gigtsygdomme</h3> <p>Slidgigt er den hyppigste årsag til, at man behandler med skulderprotese. Man tilbyder først en skulderprotese, når man har haft smerter i mange måneder, som både er der i hvile og ved funktion. Derudover skal man have forsøgt at tilpasse sine aktiviteter med skulderen og forsøgt genoptræning af skulderen. Optræning af skulderen hjælper ofte på smerterne og kan være med til at undgå eller udskyde en operation. </p> <h3>Styresene gigt (rotator cuff artropati): </h3> <p>Styresene gigt er en sygdom, hvor der er gigt i skulderleddet, som skyldes store kroniske skader på styresenerne (rotator cuffen). Ved denne gigtform mangler styresenerne, og funktionen er ofte meget begrænset. Operation med indsættelse af skulderprotese tilbydes, som ved slidgigt, når der er smerter ved funktion og i hvile. </p> <p>Til trods for at senerne mangler, kan genoptræning stadig reducere smerterne og bedre bevægeligheden. Hvis træning og tilpasning af aktiviteter ikke er tilstrækkeligt, kan man behandle med en omvendt skulderprotese. </p> <p>Der er andre mere sjældne lidelser, hvor skulderleddet er ødelagt, som kan kræve behandling med skulderprotese, herunder sammenfald af ledhovedet. </p> <h2>Hvilke protesetyper findes der?</h2> <p>Der findes to forskellige proteser - en anatomisk skulderprotese og en omvendt skulderprotese. Begge proteser giver et pålideligt resultat. Kirurgen vælger den protese, som vil give dig det bedste resultat.</p> <h3>Anatomisk skulderprotese</h3> <p>Den anatomiske skulderprotese ligner en almindelig skulders anatomi. Den består af en ledskål, der er lavet af et meget holdbart plastik materiale (polyethylen) og et ledhoved, som er lavet af rustfrit stål. Protesedelene kan forankres til knoglen på forskellige måder, og metoden vælges ud fra forskellige parametre, som kirurgen vurderer under operationen. En anatomisk protese kan kun bruges, hvis alle styresenerne er normale.</p> <p><img src="0527A2D9811F4EBF8B7D0FF40FB44881-270-312" style="height:312px; width:270px;" title="Røntgenbillede af anatomisk skulderprotese" alt="Røntgenbillede af anatomisk skulderprotese" height="312" width="270"><br> Røntgenbillede af anatomisk skulderprotese, der viser ledhovedet, der er forankret ned i knoglen på overarmen. Ledskålen er lavet af plastik og ses derfor ikke på røntgen, fraset en lille clips, der er indlejret i plastikken.</p> <h3>Omvendt skulderprotese</h3> <p>Ved den omvendte skulderprotese har man byttet om på ledhoved og ledskål. Det vil sige, at ledkuglen fæstnes på skulderbladets ledskål, og ledskålen placeres på overarmsknoglen. Denne type protese bruges i tilfælde, hvor styresenerne i skulderen er ødelagte, som for eksempel ved styresene gigt. Ved at vende protesen om, er der ikke behov for styresenerne til at løfte armen, og man kan i stedet kompensere ved at bruge skulderrundings musklen (deltoideus). </p> <p>Den omvendte skulder er også førstevalget ved komplicerede skulderbrud og følger efter skulderbrud. Ledskålen sidder nu forankret ned i overarmsknoglen.</p> <h2>Er der risiko ved operationen?</h2> <p>Der er altid en risiko for, at der kan opstå en komplikation i forbindelse med en operation. Ved operation for skulderprotese er risikoen lille, og man anslår den til at være under 1 %.</p> <p>Der er dog sjældne, men alvorlige komplikationer, som kan betyde, at du skal opereres igen, eller at du får varigt tab af funktion. De alvorlige komplikationer indbefatter </p> <ul> <li>Infektion omkring protesen</li> <li>Skade på en nerve</li> <li>Ledskred af protesen </li> <li>Knoglebrud omkring protesen</li> </ul> <h2>Efter operationen</h2> <p> Efter operationen skal du aflaste armen i armslynge i 3-6 uger.</p> <p>Du bliver tilbudt et genoptræningsforløb ved en fysioterapeut. Det er vigtigt, at du går gradvist frem. I starten er det særlig bevægeøvelserne, der er vigtige, for at du får så god en bevægelighed som muligt.</p> <p>Smerterne forsvinder gradvist ,og efter 3 måneder er de fleste patienter tilfredse med resultatet. Der går dog op til et helt år, før skulderen er så god, som den kan blive.</p> <p>Du kan køre bil, når du kan køre forsvarligt. Det vil typisk være efter ca. 8 uger.</p> <h3>Sygemelding efter skulderprotese operation</h3> <p>De fleste personer, der behandles med en skulderprotese, er pensionister.</p> <p>Det er sjældent, at man kan komme tilbage til fysisk hård arbejde efter en skulderproteseoperation. </p> <p>Sygemeldingsperioden ved kontorarbejde er 2-6 uger, og det afhænger meget af ens arbejdsopgaver og smertetilstand. </p> <h2>Hvor gode er operationsresultaterne?</h2> <p>Personer, der behandles med en skulderprotese på baggrund af en gigttilstand, kan forvente en høj grad af smertefrihed og en rimelig funktion af skulderen. De fleste kan klare almindelige daglige aktiviteter med husligt arbejde og havearbejde.</p> <p>Personer, der behandles på baggrund af skulderbrud eller følger efter skulderbrud, har ofte et dårligere resultat. Målet med behandlingen er smertefrihed, men ofte er funktionen efterfølgende dårlig. </p> <p>I Danmark registreres alle operationer, hvor der indsættes skulderprotese. Data fra registret viser, at ca. 90 % af patienterne har den samme protese efter 10 år. </p> <h3>Patienttilfredshed</h3> <p>Efter 1 år vurderer patienterne resultatet af deres proteseoperation. Man kan se, at resultaterne bliver bedre år for år. </p> <p>Gennemsnitsresultatet efter 1 år var: </p> <ul> <li>85 % for protese pga. slidgigt</li> <li>60 % for protese pga. skulderbrud </li> <li>70 %
