Lokalbehandling kan efter biopsi overvejes ved: </p> <ul> <li>unifokal noninvasiv tumor af lav malignitetsgrad</li> <li>bilateral sygdom</li> <li>nyreinsufficiens</li> <li>énnyrede patienter</li> <li>svær komorbiditet</li> </ul> <p>2. Radikal nefroureterektomi tilbydes til patienter ved:</p> <ul> <li>high grade tumor</li> <li>multifokal tumor</li> <li>billeddiagnostik mistanke om invasion</li> <li>tumor der er for stor til lokalbehandling</li> </ul> <h2>Henvisning</h2> <ul> <li>Patienter med makroskopisk hæmaturi uden anden åbenbar årsag (kræftpakkeforløb)</li> <li>Gentaget stikshæmaturi hos patienter >60 år kombineret med symptomer - f.eks. blære-/flankesmerter, palpabel udfyldning eller vandladningsgener (kræftpakkeforløb)</li> <li>Patienter < 60 år med urinstikshæmaturi og relevante symptomer kan tilbydes urologisk udredning, men ikke i kræftpakkeforløb</li> <li>Palpabel udfyldning </li> <li>Patienter med billeddiagnostisk mistænkt kræft i ureter/nyre henvises i kræftpakkeforløb også uden hæmaturi</li> </ul> <h2>Opfølgning</h2> <h3>Plan</h3> <ul> <li>På grund af høj risiko for lokalt recidiv eller urothelsygdom i blæren, er langtidsopfølgning af disse patienter vigtig</li> <li>Der bør tilbydes screening med ultralydsscanning, urinstix og urincytologi som minimum en gang årligt fra 35 årsalderen hos patienter med MSH2 mutationen ved HNPCC</li> <li>Alle patienter med 1. gangs tilfælde øvre uroteltumorer bør uanset invasion undersøges for MMR (Mismatch Repair) protein - dvs. immunhistokemisk undersøgelse for MLH1, MSH2, MSH6 og PMS2 </li> <li>Patienter med høj risiko for tilbagefald efter nefroureterktomi (min. pT3 og/eller N+) samt PD-L1 positiv tumor bør tilbydes adjuverende Nivolumab samt følges med CT-skanning hver 3. måned i et år efter opstart af immunterapi. Herefter årligt i 3 år. Bør følge 4-8-12 regime med cystoskopi</li> </ul> <h4>Lokalbehandlet solitær noninvasiv tumor af lav malignitetsgrad:</h4> <ul> <li>Senest 4 måneder. efter behandlingen foretages cystoureteronefroskopi</li> <li>Uden påviseligt recidiv: <ul> <li>Fornyet cystoureteronefroskopi efter yderligere 8 og 12 måneder </li> <li>Fra år 3-5 kontrolleres årligt med CT-urografi samt cystoskopi - patienten kan herefter afsluttes</li> </ul> </li> <li>Ved lokal recidiv bør nefroureterectomi overvejes</li> </ul> <h4>Radikal nefroureterektomi</h4> <ul> <li>Kontrolcystoskopi foretages i henhold til 4-8-12 regimet i op til 5 år</li> <li>Ved invasive tumorer suppleres med CT af thorax og abdomen i op til 3 år (4, 12, 24 og 36 måneder)</li> <li>Der er ikke indikation for rutinemæssig CT-urografi af det kontralaterale system</li> </ul> <h2>Sygdomsforløb, komplikationer og senfølger samt prognose</h2> <h3>Sygdomsforløb</h3> <ul> <li>Uden radikal kirurgisk behandling progredierer sygdommen og bliver uhelbredelig</li> </ul> <h3>Komplikationer og <a href="~/link.aspx?_id=471CA46BE82C400E9D63538E9BC4CFBC&_z=z">senfølger</a></h3> <ul> <li>Postoperativ blødning, urinvejsinfektion og strikturdannelse er de vigtigste komplikationer til endoskopisk behandling</li> <li>Brug af laser kan reducere risiko for strikturer</li> <li>Udsæd af tumorceller gennem nefrostomi-kanalen er en risiko ved perkutan behandling</li> <li>Efter kræftbehandling kan patienterne opleve træthed, koncentrationsbesvær, søvnbesvær og angst</li> </ul> <h3>Prognose</h3> <ul> <li>Tumorstadium (TNM) og differentieringsgrad (WHO-klassifikation - 5.udgave) er de vigtigste prognostiske faktorer for uroteliale tumorer i øvre urinveje</li> <li>Fem-årsoverlevelsen ved sygdomsstadie pT2-3 er 50 % mod <10 % ved pT4 tumorer</li> <li>Ikke invasive Ta-tumorer med lav malignitetsgrad kan hos udvalgte patienter behandles gentagne gange med endoskopiske procedurer</li> <li>Fem-årsoverlevelsen ved metastatisk sygdom er <10%</li> </ul> <h2>Baggrundsoplysninger</h2> <h3>Definition</h3> <ul> <li>Urotelderiverede tumorer opstået i de øvre urinveje (nyrepelvis og ureter)</li> <li>Som ved blæretumorer skelnes mellem præmaligne ikke-invasive Ta-tumorer og invasive T1-4 tumorer</li> </ul> <h3>Forekomst</h3> <ul> <li>Der diagnosticeres årligt ca. 150 nye patienter med tumor i de øvre urinveje, 65 % af disse er mænd</li> <li>Tilstanden ses sjældent før 40-års alderen og hyppigst i 60- og 70-årsalderen</li> <li>Uroteliale tumorer i nyrepelvis og ureter udgør 15 % af samtlige uroteliale tumorer, mens 85 % udgår fra urinblæren </li> <li>Uroteliale tumorer udgør over 90 % af de tumorer, som opstår i urotelet i øvre urinveje</li> <li>2/3 af tumorerne lokaliseres i nyrepelvis og 1/3 i ureter</li> <li>1/3 af tumorer i øvre urinveje optræder multifokalt</li> </ul> <h3>Ætiologi og patogenese</h3> <ul> <li>Urotelderiverede tumorer udgår fra overgangsepitelet (urotel), som beklæder urinvejene fra nyrebækkenet til den ydre del af uretra</li> <li>Uroteltumorer vokser typisk papillært og vil kunne sprede sig ved direkte indvækst i nyreparenkymet og omliggende strukturer eller ved spredning gennem vaskulære eller lymfatiske baner</li> </ul> <h3>Disponerende faktorer</h3> <ul> <li>Cigaretrygning er den største risikofaktor for uroteltumorer</li> <li>Andre risikofaktorer udgøres af eksponering for benzenderivater, aromatiske aminer, anilinfarvestoffer, petrokemikalier, kul, tjære, asfalt og fenacetin</li> <li>Kronisk inflammation ved f.eks. urinvejskonkrementer og recidiverende øvre urinvejsinfektioner udgøre en mulig risikofaktor i lighed med samme risiko under lignende omstændigheder i urinblæren</li> <li>Senbivirkning ved anvendelse af alkylerende kemoterapeutika som
